[Monthly Support] Tier-based system of support

Instead of dollar-based systems of support, move to a tier-based system. Allow creators to add tiers (i.e bronze at $2/mo, etc.) and allow subscribers to choose from a list of tiers. Also allow subscribers who don't want perks to put in a raw dollar amount, or auto-assign to the closest tier underneath their selected amount.
